You attack the enemy at the same time when they are attacking you. Instead of performing a Block Reaction - you attack the enemy at the same time. During your attack, you roll 3d20 and take the highest die. Then whoever attack deals higher damage - deals the damage difference to the opponent. 

This maneuver only works against Melee Attacks.
